Late Order Form Announcement and Cap & Gown Individual Order Information Please follow these instructions and guidelines. Please note – this website provides you with our order form, which can be mailed in to us. This website will submit the order for you, if you give credit card information. If you do not pay by credit card, the order will not be processed, until we receive it in the mail with payment. 1. Enter the information requested below. Check the package you wish to order with your space bar. Items are no longer available individually, with the exception of the cap and gown unit. Enter the student’s height and weight to order a cap, gown and tassel unit. 2. Review your order. If it is correct, choose submit order. If you do not pay by credit card, print the order and mail it to the address on the bottom of the form. The order will not be processed, unless we receive payment in full. 3. Make cashier’s check or money order payable to Larry DeWitt. Full payment is required – cashier’s check, money order or credit card. 4. Orders must be received in time to process before graduation. Orders will be shipped to your home at the end of May, with the exception of cap and gown units, which will be delivered directly to the school. 5.
